Chronic Kidney Disease (CKD) and end stage renal disease (ESRD) are serious and costly diseases that consume nearly 25 percent of the Medicare budget. The highest rates of CKD, which affects more than 20 million adults, are seen in individuals over 60 years of age. Diabetes and hypertension are the most frequent causes of CKD in the United States. In 2013, there were 2,287 patients in chronic renal replacement therapy in Delaware.
Please join us as we work together to address CKD and ESRD through this unique initiative which engages stakeholders in Patient-Centered Outcomes Research (PCOR) and Community Engaged Research (CER).
